Payment + Fees

Home Birth

Our total fee for prenatal care, labor and birth and postpartum care is $6000. We accept self- pay and Vermont Medicaid. For those with private insurance, we work with a biller for client reimbursement. See below for more information.

Our fee includes:

• $600 deposit at the initial prenatal visit to hold your spot in our practice

• The total fee includes the $600 assistant fee, common with other midwifery practices

• The remainder of the fee must be paid by 36 weeks gestation, unless we have a payment plan or barter in place. We will terminate care / transfer care to another provider if not paid in full by 36 weeks.

We encourage those with access to financial wealth (including access to family wealth) to pay our full fee. Please use the image below to help determine where you fall on the sliding scale.

Those with the financial resources are encouraged to contribute to our equity fund (see below) and pay an additional 10-50% of the full fee to support accessible midwifery care for all people.

We have payment plan options available, that can accommodate your financial realities. Please be in touch to discuss a payment plan.

Sliding Scale for Self-pay clients

For those with less financial resources, please use the image above to place yourself on the sliding scale. We accept a limited number of clients at the reduced sliding scale rates.

Sliding Scale Option 1

$4900 total fee for prenatal, labor and birth and postpartum care. This is appropriate for those who self-identify with the middle bottle above.

Sliding Scale Option 2

$3800 total fee for prenatal, labor and birth and postpartum care. This is appropriate for those who self-identify with the right-most bottle above.

Insurance

We accept Medicaid (Green Mountain Care and Dr. Dynasaur). Medicaid does not cover all of our expenses, so Medicaid clients are asked to pay an out-of-pocket expense for non-billable services. Our sliding scale for out-of-pocket expenses is $1800 - $3000. See Green Bottle image above to self-determine where you fall on the sliding scale, and ask the midwives for more information about this.

For those with private insurance, Threshold Community Midwives are considered out-of network providers. Clients with private insurance will be considered self-pay clients and are responsible to pay our full fee by 36 weeks gestation. Many PPO-type insurance plans will provide some amount of reimbursement for home birth midwifery care with out-of-network providers. We can refer you to our medical biller who will do a verification of your benefits (fee of $45) before you start care so you have a sense of what your insurance company may reimburse you for. At the end of your care, the biller will submit a claim to your insurance company. Depending on your plan your insurance company will reimburse you for part of what you paid us for care.

Other home birth midwifery care, most insurance companies will not provide reimbursement or coverage for any additional services we offer. We’d love to see health insurance change to support client needs and necessary providers of all kinds.

fertility + insemination

Fertility Consultations

• 1.5 hour initial consultation visit with 3 months of phone / email support: $150

• 1 hour on-going fertility consultations: $100

Intrauterine Inseminations (IUI)

Package Fee: $600 for frozen sperm, $800 for fresh sperm (includes a sperm washing process). Includes:

• 1.5 - hour fertility consult with 3 months of phone / email support

• 2 IUIs (can be in one cycle or two)

IUI non-package Fees

• Frozen sperm:

  • $300 - $450 sliding for 1 IUI

  • $150 for additional IUI per cycle

• Fresh sperm:

  • $400 - $550 sliding for 1 IUI

  • $250 for additional IUI per cycle

Well Body care

These services are included in your total birth fees + any labs needed (insurance covers labs in most cases). Contact us for more information.
